1 tube of choice - I used to wonderful artwork of Renee Biertempfel here
Font of choice - I used Felt Tip here

This tutorial was written assuming you have a working knowledge of PSP.

~♥~ Let’s get started ~♥~

Open up the template.
Shift + D.
Close out the original.
Delete the credit layer.
Canvas Size
700 x 700
Flood fill the background layer white.

Highlight Circle layer.
Select All/Float/Defloat
New Raster layer.
Copy and paste a dark colored paper of choice Into Selection.
Selections/Modify/Contract by 8
Tap the delete key.
Selections/Modify/Expand by 2
New Raster layer below the “new frame” you just made.
Copy and paste a paper of choice Into Selection.
Deselect.
Delete the template layer.

Highlight Thin Rectangle layer.
Select All/Float/Defloat
Copy and paste a paper of choice as a new layer.
Selections/Invert
Tap the delete key.
Deselect.
Delete the template layer.
Give it a slight inner bevel:

Highlight Thin Rectangle 2 layer.
Select All/Float/Defloat
New Raster layer.
Copy and paste a paper of choice Into Selection.
Tap the delete key.
Deselect.
Delete the template layer.
Give it the same slight inner bevel.

Highlight Thin Rectangle 3 layer.
Select All/Float/Defloat
New Raster layer.
Copy and paste a paper of choice Into Selection.
Tap the delete key.
Deselect.
(I used my Target Tool and changed the color of the paper to a color that matches my tube.)
Delete the template layer.
Give it the same slight inner bevel.

Highlight Circle Background.
Select All/Float/Defloat
Copy and paste a paper of choice as a new layer.
Selections/Invert
Tap the delete key.
Deselect.
Delete the template layer.

Highlight Circle 1 layer.
Select All/Float/Defloat
New Raster layer.
Copy and paste a paper of choice Into Selection.
Tap the delete key.
Deselect.
Delete the template layer.
Give it the same slight inner bevel

Repeat the above step with Circle 2.

Add you tube as a new layer.

Add your embellishments. I used:
Branch 4: Resize 70%
Bridge 1
String 1: Resize 50%
Bow 2: Resize 50%
Leaf 7: Resize 40%
Leaf 3: Resize 40% Free rotate left 40 degrees
Leaf 2: Resize 40% Free rotate left 40 degrees
Branch 3: Resize 35%
Leaf 6: Resize 35%

Highlight your white canvas.
New Raster Layer.
Select All.
Copy and paste a paper of choice Into Selection.
Deselect.
Apply the mask.
Merge Group.

Add your copyrights.

Add your name.
Give it an inner bevel.

Resize all layers 80%.
